POSITION SUMMARY:
Provides direction, coordination, supervision and implementation of professional and supportive services to clients in a safe, cost-effective manner.
Reports to: Administrator
QUALIFICATIONS:
- Graduate of an accredited nursing program with a current license as a Registered Nurse in the state(s) of practice.
- Baccalaureate degree in nursing or related field preferred.
- Minimum of two (2) years experience in nursing with a combination of acute care and home health care/public health preferable.
- Prior supervisory/management experience.
- CPR certified.
- Excellent written and verbal communication skills and strong interpersonal skills.
ESSENTIAL FUNCTIONS/AREAS OF ACCOUNTABILITY:
Planning
- Plans, directs and participates in delivery of professional services to home care clients.
- Participates in the organization’s strategic planning activities.
- Assists in the planning and development of agency structure, clinical and administrative policies and procedures and in the annual operating and capital budgets.
- Focuses services toward diagnosis-specific client care needs.
- Seeks consultant expertise as needed to ensure agency strength.
Coordination of Services
- Coordinates and directs programs and staffing to promote quality services for clients.
- Establishes and implements systems to inform clients and their families of the scope and nature of home health services and community resources.
- Coordinates services to ensure the availability and competency of staff to respond to agency referrals and provide assessment visits within 48 hours of referral.
- Provides education and training related to clinical practice issues and regulation and reimbursement changes.
- Maintains liaison with attending physicians.
- Establishes and implements priorities for services within the limitations of available resources.
Human Resource Management
- Participates in the hiring and orientation of personnel.
- Identifies the standards of practice for agency personnel and communicates these standards to them.
- Implements systems to assure compliance with all state, federal and local laws.
- Provides staff development and in-service education programs to ensure competencies of staff and to meet regulatory requirements.
- Provides staff direction through general meetings, timely personnel evaluations and indentified performance expectations.
- Provides leadership to the clinical management staff.
- Communicates effectively with all members of the interdisciplinary team.
Reimbursement for Services
- Identifies admission criteria for new clients and evaluates eligibility for home health care benefits through third-party payers. Provides clients and families with information about covered services as well as those services requiring private pay.
- Conducts monthly review of records to ensure continuing home health care client eligibility for covered services.
Policies and Procedures
- Assures compliance with the requirements of state licensure and Medicare certification through policy and procedure development, staff education and ongoing monitoring of activities.
- Recommends new policies/changes in current policies to improve quality of client care and the efficiency and effectiveness of services.
- Reviews and updates client care procedures annually.
Quality Improvement
- Establishes Quality Improvement teams to address priorities for improvement activities.
- Identifies data collection requirements and reports to evaluate the effectiveness of the programs and to ensure quality of client care.
- Identifies systems to recognize client needs, respond to client needs and to measure the outcomes of agency interventions. Utilizes this data to provide direction for agency improvements.
- Serves on the Professional Advisory Board.
Other
- Acts on behalf of the agency Administrator in his/her absence.
- Performs other related duties and responsibilities as deemed necessary.
