POSITION SUMMARY:

Provides direction, coordination, supervision and implementation of professional and supportive services to clients in a safe, cost-effective manner.

Reports to: Administrator

QUALIFICATIONS:

  1. Graduate of an accredited nursing program with a current license as a Registered Nurse in the state(s) of practice.
  2. Baccalaureate degree in nursing or related field preferred.
  3. Minimum of two (2) years experience in nursing with a combination of acute care and home health care/public health preferable.
  4. Prior supervisory/management experience.
  5. CPR certified.
  6. Excellent written and verbal communication skills and strong interpersonal skills.

ESSENTIAL FUNCTIONS/AREAS OF ACCOUNTABILITY:

Planning

  1. Plans, directs and participates in delivery of professional services to home care clients.
  2. Participates in the organization’s strategic planning activities.
  3. Assists in the planning and development of agency structure, clinical and administrative policies and procedures and in the annual operating and capital budgets.
  4. Focuses services toward diagnosis-specific client care needs.
  5. Seeks consultant expertise as needed to ensure agency strength.

Coordination of Services

  1. Coordinates and directs programs and staffing to promote quality services for clients.
  2. Establishes and implements systems to inform clients and their families of the scope and nature of home health services and community resources.
  3. Coordinates services to ensure the availability and competency of staff to respond to agency referrals and provide assessment visits within 48 hours of referral.
  4. Provides education and training related to clinical practice issues and regulation and reimbursement changes.
  5. Maintains liaison with attending physicians.
  6. Establishes and implements priorities for services within the limitations of available resources.

Human Resource Management

  1. Participates in the hiring and orientation of personnel.
  2. Identifies the standards of practice for agency personnel and communicates these standards to them.
  3. Implements systems to assure compliance with all state, federal and local laws.
  4. Provides staff development and in-service education programs to ensure competencies of staff and to meet regulatory requirements.
  5. Provides staff direction through general meetings, timely personnel evaluations and indentified performance expectations.
  6. Provides leadership to the clinical management staff.
  7. Communicates effectively with all members of the interdisciplinary team.

Reimbursement for Services

  1. Identifies admission criteria for new clients and evaluates eligibility for home health care benefits through third-party payers. Provides clients and families with information about covered services as well as those services requiring private pay.
  2. Conducts monthly review of records to ensure continuing home health care client eligibility for covered services.

Policies and Procedures

  1. Assures compliance with the requirements of state licensure and Medicare certification through policy and procedure development, staff education and ongoing monitoring of activities.
  2. Recommends new policies/changes in current policies to improve quality of client care and the efficiency and effectiveness of services.
  3. Reviews and updates client care procedures annually.

Quality Improvement

  1. Establishes Quality Improvement teams to address priorities for improvement activities.
  2. Identifies data collection requirements and reports to evaluate the effectiveness of the programs and to ensure quality of client care.
  3. Identifies systems to recognize client needs, respond to client needs and to measure the outcomes of agency interventions. Utilizes this data to provide direction for agency improvements.
  4. Serves on the Professional Advisory Board.

Other

  1. Acts on behalf of the agency Administrator in his/her absence.
  2. Performs other related duties and responsibilities as deemed necessary.
