POSITION SUMMARY:

Manages, supervises, coordinates, evaluates and develops client care teams to ensure quality care delivery and appropriate case management within the agency scope of services and policies, the state, federal and local laws and Nurse Practice Act.

Reports to: Director of Home Care

Supervises/Directs: RN Case Managers

Provides direction to team members and support staff.

QUALIFICATIONS:

  1. Graduate of an accredited school of professional nursing. BSN and PHN certification preferred.
  2. Current license as a Registered Nurse in the state(s) of practice.
  3. Minimum of two (2) years nursing experience in an acute hospital setting or equivalent experience with one (1) year of supervisory experience.
  4. Excellent verbal and written communication skills and strong interpersonal skills.
  5. Knowledge of home care federal and state regulations.
  6. Knowledge of reimbursement sources and documentation requirements within home health care.

ESSENTIAL FUNCTIONS/AREAS OF ACCOUNTABILITY:

Coordination of Services

  1. Manages and directs a team of Nurse Case Managers to ensure delivery of safe, effective and appropriate home care services.
  2. Develops and implements plans in response to the needs of the team or client service needs.
  3. Collaborates with the Director in establishing operating budget needs and priorities for the department.
  4. Meets with Case Managers on a regular basis to provide guidance and information related to specific issues.
  5. Reviews monthly productivity information of team members.
  6. Coordinates 24-hour agency coverage by Registered Nurses. Evaluates quality of on-call services.
  7. Provides on-call backup for Case Managers, as needed.
  8. Develops working relationships with other health care professionals in the community and clients’ families to identify resources available and to ensure access of information to clients.

Staff Development/Professional Growth

  1. Provides support and direction to agency staff, other health care professionals, clients and families related to appropriate and available health care resources.
  2. Educates agency staff on clinical services, policies and procedures as needed.
  3. Facilitates problem-solving sessions to enable Case Managers and other staff to resolve client/reimbursement source issues.
  4. Stays current on the available community resources, health care costs and industry trends through self-education and access to outside educational opportunities.

Regulation/Compliance

  1. Ensures accuracy, completeness and timeliness of clinical documentation in accordance with agency policies and procedures, regulatory requirements and industry standards.
  2. Monitors open and closed charts regularly.
  3. Reviews agency policies and procedures and revises as needed.
  4. Provides educational programs and information regarding appropriate documentation practices.

Quality Improvement

  1. Provides leadership to team and support staff in identifying agency/client needs and opportunities for quality improvement.
  2. Assists with marketing, public relations and discharge planning by participating in departmental meetings.
  3. Assists quality improvement teams with data collection for the Quality Improvement Plan established by the agency.
  4. Identifies and implements changes in clinical/operational practice based on the findings of the Quality Improvement Program.

Human Resource Management

  1. Provides human resource management to achieve quality services and positive employee relations.
  2. Interprets and enforces human resource policies and procedures in a fair and consistent manner.
  3. Conducts timely performance evaluations in accordance with agency policy.
  4. Provides on-site supervision per agency policy and as needed to determine staff competency and respond to educational and developmental needs.
  5. Follows agency guidelines for disciplinary actions. Documents all disciplinary actions in accordance with human resources policies and legal guidelines.
  6. Monitors employee turnover, overtime and absenteeism. Takes appropriate actions to address problems/issues.
  7. Collaborates with Director and other clinical management staff to ensure proper staffing of qualified, competent staff.

  1. Promotes personal safety and a safe environment for clients and coworkers.
  2. Demonstrates knowledge of safety/infection control practices by compliance with policies and procedures and regulatory requirements.
  3. Assesses safety of environment and takes the initiative to help prevent accidents and promote safety.
  4. Recognizes and responds appropriately to potential unsafe situations.
  5. Performs other duties as assigned.
